Doctor Discussion Guide - Questionnaire

Doctor Discussion Guide is an interactive wizard tool designed as a questionnaire with structured questions and answers.

After completing the questionnaire, the user is directed to a summary page.

The summary displays all the user’s responses alongside informative content. The summary can be: Saved as a PDF, Printed and/ or Sent via email.

The questionnaire is organized into steps. Each step contains a single question with answer options, which may include: Single-choice, Multiple-choice, Free-text responses.

Once the first step is completed, the user can navigate backward using the Previous button. Additionally, progress indicators for earlier steps function as navigation buttons.

The final step leads to the Results Summary page, where the user can download the summary as a PDF or send it by email.

On the summary page, a Start Over button is available. Clicking this button resets all answers and returns the user to the beginning of the questionnaire, allowing them to start again.
