Form - All the options for display

Forms are used to fill in the user's details on topics, such as: contact us, report of side effects/ adverse events and more.
The submit button sends the form details to the website owner's email.
The end customers will also be able to receive an email notification that the form has been submitted.


Variety of filling fields:

  • Free text
  • Single selection by drop-down list, check-box list, radio buttons
  • Multiple selections by check-box list, radio buttons
    In addition, the check box/ radio buttons can be align to top, in the case of multiple lines of text.
  • Drop-down selection from a manual list
  • Drop-down Auto-suggest
  • Branching (condition) fields
  • Digital signature by hand/mouse
  • Mandatory fields/ typing according to patterns.

In addition,

  • The label name, in each field, can be in any color you choose.
  • It is possible to add free text between the form fields, to allow section titles/ split into sections.

Submit the form and sending to Email - 

  • All data filled out on the form, including all files attached by the user (if any), will be sent to the email.
  • If the form includes the digital signature field, then a copy PDF file of the form will be sent to the email, including the user's signature.
  • The form details can be sent to multiple receivers, according to a topic chosen by the user in one of the fields (drop-down list, check-box list, radio buttons).
  • The end customers will see an indication, after submitting the form, about the successful sending. Additionally, it is optional to receive a "Thank you for contacting us" email.
